DO NOT FILL OUT FORMS BELOW IF YOUR CHILD(REN) HAVEN'T TAKEN OUR REQUIRED TESTING AND FILLED OUT AN APPLICATION.

Once you've completed testing and have been accepted, you may 1) print & fill out these forms at home or 2) visit the school office for already printed out forms. PLEASE COMPLETE & RETURN FORMS BEFORE THE 1st DAY OF SCHOOL TO THE SCHOOL OFFICE. For any questions, see the school office.

For ohana with more than (1) keiki, you may write all of their names on one form for convenience or print individual forms for each child.
